Unsubscribing using Forminator and MailChimp

I am using Forminator Pro and MailChimp. Setting up a form (with name and email) and integrating it with MailChimp was a breeze. Very cool. However, I’m looking for step by step instructions to set up an unsubscribe form with an email field that sends the email to MailChimp so it gets unsubscribed.

I read the MailChimp article at https://mailchimp.com/help/about-unsubscribes/, but I’m new-enough to the WordPress world that I don’t understand how to do this.
